This report can be completed by the victim or bank or credit union or law enforcement.
The form is used when a victim had their car broken-in or burglary of home/business and their checkbook was stolen. Just reporting it to their bank or credit union will not always get the information out to retailers fast enough. We will send to our member merchants and financial institutions quickly. The sooner the report the better.
This avoids the victim having to deal with collection agencies and completing affidavits of forgeries when the checks are decline.
